Our Community Support Service offers support to individuals affected by Traumatic Brain Injuries ( primarily).

Through this service our Personal Assistants offer one to one person-centred support to survivors in their homes and in the community.

Community support is focused on an individual’s unique needs, goals and aspirations. Our aim is to maximise a person’s potential and allow them to live as independently as possible.

We will work with the client, family, case managers and carers to agree achievable goals, develop their independence and help them integrate into their local community.

Our service is available via referral from a Case Manager, Social Work Practitioner or through private funding.

Depending on individual needs, our service can offer:

• Supporting a person as they seek to come to terms with their brain injury
• Facilitate the delivery of a tailored support plan with strategies designed to promote independence
• Supporting an individual through the rehabilitation process to re-enable them
• Working alongside health and social care professionals helping survivors to benefit from various therapeutic activities.
• Enabling greater social integration through the pursuit of hobbies and leisure activities
• Providing respite for family
• Household management
• Developing budgeting skills and better monetary awareness
• Providing benefits advice, support with form filling and understanding correspondence
• Travel training to various locations
• Supporting a person to engage in appropriate activities related to education and voluntary work

We specialise in: Working with clients with Acquired Brain Injuries. Our primary focus for this service are clients who have a Traumatic Brain Injury or a brain injury caused by medical negligence.

We are focused on: Re-enablement and working with clients to ensure they achieve for themselves as much as they can safely do. We will work with them to build confidence and re-learn lost skills through an agreed person-centred support plan.

We work with clients who: Have a medical diagnosis, are over 18 years of age and live within the Black Country Area – the 4 boroughs of Dudley, Sandwell, Walsall and Wolverhampton.

We do not: Provide any personal care, domiciliary care or administration of medication in our support packages. We do not provide 24-hour support packages.

We will: Charge in accordance with the complexity of the clients needs. Our basic charges are between £20-25 per hour.

We aim to: Reduce support packages over a period of time. To enable clients to do more for themselves and gain confidence in their own abilities. This is not always possible for all clients as some will always need support to function in the community.
